Department of Health Tasmania in Hobart is seeking a Clinical Nurse Specialist – Discharge Planner to support secure, timely, and coordinated discharges within an acute mental health setting. This role operates within a multidisciplinary team to ensure person-centred, recovery-oriented discharge planning and to liaise with NDIS providers and accommodation services for smooth transitions.

The position is permanent, part time, day worker (60.8 hours per fortnight), with a pro rata salary range of
