An exciting opportunity is available to become an integral member of our team as a
Social Worker
on a
permanent, full-time
basis.
n
In this role, you will provide high-quality, recovery-focused psychosocial care to patients, supporting their mental health, wellbeing, independence and connection with family, community and other support services.
n
Working across the community and onsite clinic, you will collaborate closely with patients, families, carers and the multidisciplinary team to provide person-centred care, support recovery goals and facilitate safe and effective transitions across services.
Responsibilities: n
n
Provide comprehensive psychosocial assessments, risk assessments and evidence-based Social Work interventions for patients experiencing a range of mental health presentations.
n
Support patients to identify and achieve individual recovery goals, build independence and strengthen their connection with family, community and social supports.
n
Provide therapeutic interventions including brief interventions, supportive counselling, motivational interviewing and trauma-informed practice, where appropriate.
n
Work collaboratively with the multidisciplinary team to develop and review individualised care plans and facilitate safe, coordinated transitions and discharge planning.
n
Provide advocacy, education and practical support to patients, carers and families.
n
Support patients and families experiencing adjustment, loss, grief,



bereavement and the social and psychological impacts of mental illness.
n
Identify and respond appropriately to risk and safeguarding concerns, including child abuse, elder abuse and domestic and family violence.
n
Develop strong partnerships with community organisations and external services to connect patients with appropriate health, housing, employment, education and social supports.
n
Contribute to multidisciplinary case discussions, clinical reviews and collaborative care planning.
n
Maintain high standards of professional practice, clinical documentation, confidentiality and ethical decision-making.
n
About You To be successful in this role, you will meet the following criteria: n
n
Bachelor of Social Work (or recognised equivalent).
n
Minimum 4 years experience
as a Social Worker
n
Eligible for full membership of Australian Association of Social Workers.
n
Demonstrated ability to utilise a variety of Social Work interventions, inclusive of effective problem solving andethical decision -making skills.
n
Demonstrated experience in Healthcare environments
n
Knowledge of community resources to meet client's needs.
n
Proven ability to work independently and effectively in a multidisciplinary team.
n




Well-developed interpersonal and communication skills (verbal and written)
n
Familiar with Microsoft Suite
n
Unrestricted NSW drivers' licence
n
Personal vehicle that may be used for work purposes
