You will deliver person-centred support within the Support at Home program, coordinating with participants, families and providers to promote protected, independent living aligned to goals.

Salary: $93,670.78–$100,783.94 per annum plus 12% superannuation (pro-rata for part time). This role emphasizes compassionate care, budget prudence, and collaboration with health professionals
