Franklin Family Support Services – Brisbane QLD

Franklin Family Support Services is seeking a Family Support Worker to provide practical, person-centred support to children and families.
n
In this role, you will:
n
n
Meet with families to understand their needs, strengths and goals.
n
Assist in planning and delivering tailored support and education.
n
Support families with routines, parenting skills and access to community services.
n
Work with children and people with disability in home and community settings, where relevant to their support plans.
n
Liaise with social workers, support coordinators, schools and other services as needed.
n
Monitor progress and maintain transparent, accurate records.
n
Identify when a family needs additional assistance and make appropriate referrals.
n
n
We're looking for someone with relevant experience supporting families, children or people with disability; strong communication and documentation skills; and the ability to work respectfully with people from different backgrounds. Relevant qualifications,



screening checks and licence requirements should be confirmed against the final position requirements.
