You will assess, plan and coordinate care for adults with complex needs using recovery-focused, trauma-informed and culturally protected approaches.
You will work with GPs and a multidisciplinary team to deliver shared care, evidence-based interventions and coordinated support.
You will maintain records and support governance while supporting recovery journeys.
