Job Description
Primary Care Connect is seeking a Practitioner – Family Violence Victim-Survivor (FVVS) to provide triage, screening, assessment, and short-term responses to referrals, ensuring safety of victim-survivors within The Orange Door network.

Working in a multi-disciplinary team, you will identify and manage risk under the MARAM framework, coordinate with internal and external partners, and maintain compliant, privacy‐aware documentation.
