Working within local communities, you'll help increase participation, inclusion, and access to essential supports and services.
Your New Role n
As a Local Community Connector (LCC), you will support people with disabilities, their families, and carers to access appropriate services and community support.
Key Responsibilities n
n
Assisting participants with access and planning processes.
n
Providing information on disability, community, and mainstream supports.
n
Developing and monitoring community connection plans.
n
Conducting participant check-ins and supporting plan changes where required.
n
Building relationships with community organisations and service providers.
n
Supporting community engagement activities and local service delivery.
n
Maintaining accurate records and completing administrative tasks.
n
What You Will Need to Succeed n
n
Experience in disability, community services,



health, human services, or a related field.
n
Robust stakeholder engagement and customer service skills.
n
Experience working with Aboriginal and Torres Strait Islander communities.
n
Ability to manage competing priorities and solve problems effectively.
n
Strong written and verbal communication skills.
n
Experience working independently and as part of a team.
n
Proficiency using Microsoft Office and client management systems.
n
Willingness to obtain an NDIS Worker Screening Check and Working with Children Check.
n
Desirable n
n
Desirable: Knowledge of the disability sector and person-centred service delivery. Experience in regional, rural, or remote communities.
n
The Logistics n
n
Australian Citizenship required.
n
APS4 and APS5 opportunities available.
n
Hybrid working arrangements.
n
Travel within local communities may be required.
n
Driving licence preferred.
n
Community and participant home visits may form part of the role.
