Multidisciplinary teams work collaboratively to support individuals, families, and communities toward better health and wellbeing.
Key Responsibilities
Assist with health assessments, including 715 Health Checks
Support chronic disease management, care planning, and follow‐up
Assist GPs and Nurses with clinical procedures and treatment room activities
Build strong relationships with Aboriginal and Torres Strait Islander clients and community
Support clients to access health services, attend appointments, and navigate the health system
Promote culturally safe, respectful, and person‐centred care
Support health promotion, education, and preventative health initiatives
Encourage healthy lifestyles and early intervention strategies




Participate in community engagement activities and programs
Work collaboratively within a multidisciplinary team
Maintain accurate clinical records using clinical software systems
Contribute to quality improvement and accreditation activities
Identify as an Aboriginal and/or Torres Strait Islander person
Minimum Certificate III in Aboriginal and Torres Strait Islander Primary Health Care (or nearing completion)
Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Health Practitioner), or willingness to work towards
Demonstrated experience delivering primary health care services, or a strong desire to build a career in the field
Strong understanding of health issues impacting Aboriginal and Torres Strait Islander communities
Demonstrated ability to work effectively with Aboriginal and Torres Strait Islander people and communities
Robust communication and interpersonal skills
Ability to work effectively within a multidisciplinary team
Ability to manage workload and prioritise competing demands
Current Open C Class Driver Licence
Current CPR certification
Ability to obtain and maintain a National Police Check and Blue Card
Experience in an Aboriginal Medical Service or community‐controlled health organisation
Experience supporting chronic disease management and health assessments
Experience using Best Practice or similar clinical software
Additional training in health promotion, preventative health, or community engagement
Previous experience in health, aged care, disability, or community services
