Job Description
Life Without Barriers is seeking a Case Manager – Family, Based Care to support children and young people in Out of Home Care. You will coordinate care, safety, and wellbeing, assess needs, develop and review case plans, and work with families, carers and agencies to achieve positive outcomes.

The role requires a Bachelor-level qualification, experience in case management, solid communication, and a current driver's licence.
