Job Description
The City of Whittlesea is seeking a Care Partner to deliver person-centred, goal-directed care to help clients live independently in the community. This full-time role focuses on developing and reviewing annual client care plans and identifying changes in health, wellbeing and needs.

You will work with clients, carers, local agencies and council staff as part of the Case Management team, with adaptable working arrangements and a band 5 remuneration package.
