Job Description
As our Registered Nurse, you'll play a central role in the clinical oversight of client care across the Greater Sydney region. You'll conduct home visits, complete clinical and care needs assessments, and work closely with clients, families, and our support workers to ensure every care plan reflects best-practice, evidence-based nursing.

Key Responsibilities

Conduct comprehensive clinical assessments and care needs assessments for recent and existing clients in their homes
n
Develop, implement, and regularly review individualised care plans in line with Support at Home program requirements
n
Carry out scheduled home visits across Greater Sydney to monitor clinical status, wound care, medication management, and general wellbeing
n




Provide clinical oversight and guidance to support workers and care staff delivering day-to-day care
n
Identify and escalates changes in client condition, coordinating with GPs, allied health, and specialists as needed
n
Maintain accurate, timely clinical documentation in line with regulatory and organisational standards
n
Support compliance with Aged Care Quality Standards, and other relevant legislation such as the Aged Care Act
n
Provide support in incident management, risk assessments, and continuous improvement initiatives
