We're looking for an experienced Senior Case Manager ( Empowering Family Connections ) to join our Intercept Community Services team, supporting young people and their families through complex, real-life challenges.
Location: Caboolture, QLD
Role Type: Part Time | Mon to Thu | 9am - 5pm | 60 hrs fortnight
Remuneration Range: $***** - $***** ph + Super + Salary Packaging + Employee discounts
At Intercept Community Services, Empowering Family Connections program is a targeted family support program that works with young people and their families to strengthen connections and support them to prevent entry or re-entry into the child protection system.
What You'll Be Doing:
Delivering
end-to-end case management
for young people and families
Conducting
risk and needs assessments
and developing tailored support plans
Providing
counselling, mediation, advocacy, and family support
Supporting
home stability, life skills development, and transitions
Working collaboratively with
Child Safety, Youth Justice, Health, Education and other stakeholders
Facilitating
individual and group programs
Maintaining accurate
case notes, reporting, and data collection
Participating in community networks and service development initiatives
What You'll Bring:
Degree in Human Services, Social Work, Psychology or related field
Strong experience in case management within youth, family or community services
Ability to manage complex cases using therapeutic and evidence-based approaches
Confidence working with multiple stakeholders and high-risk situations
Excellent communication, documentation, and organisational skills
Current
Blue Card, NDIS Worker Screening, and QLD Driver's Licence
