An exciting opportunity within HammondCare as a
Team Leader
for Dementia Support Australia (DSA)! This leadership role offers the opportunity to manage a team of staff, ensuring high-quality service delivery for the DSA program. You will provide professional guidance, foster a supportive culture, and drive program outcomes. The role involves overseeing case management, managing resources, ensuring compliance with KPIs, and maintaining robust relationships with key stakeholders. The role n
Apply your expertise in assessment and biopsychosocial approaches, implement practical, innovative and non-pharmacological strategies to prevent, minimise or respond to changes in behaviour related to dementia.

This role is offered as a Permanent Full-time opportunity, ideally based out of our Daw Park, SA or Darwin, NT office,
with flexibility to be located at any of our offices across Australia.
Responsibilities n
n
Understand and engage with HammondCare Mission in Action, Model of Care, Case Management Framework and practice guidelines, policies and procedures.
n
Provision of professional and clinical leadership for team members
n
Ensuring timely allocation of incoming referrals to the appropriate consultant in alignment with DSA KPI's and Activity Work Plan.
n
Responsible for office environments including safety, routine maintenance and functioning equipment and ICT.
n
Oversees local use and maintenance of fleet vehicles including utilisation, electronic tracking and reporting.
n
Responsible to delegated approvals for brokerage and travel, in line with financial delegation and HammondCare Policy.
n
Ensure that the wellbeing and safety of team members is considered and proactively managed.
n
With support of Senior Team leader maintain local key stake holder relationships
n
Provide regular reporting to Regional Manager on all staff performance, service quality, operational performances and issues.
n
Recruit team members with appropriate skills for the specific roles being recruited for.
n
Effectively manage customer complaints or challenging situations as they arise, with early notification to the Regional Manager, and to the Head of National Programs when needed.
n
Work with Regional Manager to address incidents and feedback relating to the quality of the DSA Service
n
Improving your own knowledge and skills as part of the Advisory Care & Partnerships team.
n
What do we need from you? n
n
Tertiary qualifications in Nursing/ Allied Health or health- related degree.
n
Demonstrated experience and knowledge in supporting older people and carers living with dementia, with particular emphasis on psycho-social approaches to case management.
n
Ability to manage and support a team of skilled professionals across multiple sites and locations.
n
Displays strong customer service ethic and attitude
n
Ability to work on multiple tasks and projects simultaneously.
n
High level communication and interpersonal skills
n
Strong planning and organising skills with the ability to prioritise tasks to meet deadlines.
n
Strong analytical skills and attention to detail.
n
Good customer management skills for delivery of services to HammondCare and external customers
n
Strong administrative and computer skills.
n
A current Australian driver's licence and willingness to travel intra and interstate
n
Empathy and patience
n
Eligibility for registration as a Health Practitioner in Australia
n
Evidence of study and or qualifications in the area of dementia
n
Knowledge of supporting older people and carers living with dementia
n
