About us:

Welcome to our 30-bed Interim Care Unit at Sunset Ridge! We provide 24-hour care and support for people who are ready to take their next step after hospital.

Our unit has two wings: Seahorse, our secure area, and Starfish, our non-secure area.

Interim Care is a bridge to what comes next — supporting people who no longer need acute hospital care but aren't quite ready to return home. We help people prepare for their next chapter, whether that's moving to Residential Aged Care or returning to the community with NDIS supports.

About the role:

- Bring the fun!Plan, deliver and evaluate engaging activities that support patients' physical, mental and social wellbeing — including activities tailored for people living with dementia.
- Keep things running smoothly.Look after our recreation equipment and supplies, making sure everything is clean, safe and ready for action.
- Get everyone involved. Support and guide volunteers, students and visitors, while building connections with community groups to keep patients connected to the world around them.
- Make it personable. Get to know each patient through social profiles, lifestyle assessments and feedback,



using what you learn to continually improve our activities and programs.

About you:

- A relevant qualification is highly desirable — such as Leisure and Health, Diversional Therapy, Community Services, Disability Studies, or an equivalent trade-level qualification.
- Understand individual abilities, interests and needs, then turn them into meaningful and enjoyable activities.
- Confident developing lifestyle programs, including activities that support people living with dementia and other cognitive challenges.
- Understand aged care requirements, enjoy working as part of a team, embrace change and are passionate about continually improving the care and experiences we provide.

What we offer:

Total remuneration value of up to $60,579 p.a. is comprised of:

- An hourly rate between $34.53 - $36.34 p.h.
- 12.75% employer superannuation contribution
- 17.5% leave loading
