- Base salary range: $70.92 - $77.00 p.h.

About your opportunity

Join the Frailty Intervention Team (FIT) and help improve the healthcare journey of frail older people across emergency, inpatient and residential aged care settings.

As a Clinical Nurse Consultant, you will provide specialist gerontic assessment, clinical leadership and care coordination through three key service streams:

- Frailty Emergency Department Investigation (FEDI):



Partner with the Emergency Department team to identify frailty early, assess clinical risks, establish goals of care, and support timely clinical decision-making, care planning and disposition.
- FIT Triage: Provide expert telephone triage, clinical advice and care coordination for Residential Aged Care Home (RACH) residents experiencing acute health concerns, supporting access to the most appropriate care pathway.
- FIT Inpatient Consultation: Monitor admitted RACH residents, promoting continuity of care, reinforcing goals of care, integrating geriatric principles into acute care and facilitating safe, timely discharge planning.

This is an exciting chance to influence clinical management across the continuum of care, strengthen partnerships between hospital and residential aged care services, and make a meaningful difference to the outcomes, experience and wellbeing of frail older people.
