Bega Garnbirringu is a leading Aboriginal Community Controlled Health Organisation providing culturally safe, holistic and accessible health and wellbeing services to Aboriginal and Torres Strait Islander people in the Goldfields region. Our Social Support team works alongside individuals, families and the wider community to promote wellbeing, strengthen connections and support people to access appropriate services.

About the Role

We are looking for an Aged Care Remote Community Connector. The Aged Care Connector will contribute to improving access to aged care services, reducing barriers for Aboriginal and Torres Strait Islander people, increasing opportunities to receive care on Country, and supporting the growth of an Aboriginal aged care workforce.

Key Responsibilities

- Assist clients to access and navigate aged care services, including My Aged Care.
- Provide advocacy, referrals, and ongoing support throughout the aged care journey.
- Conduct client assessments, support planning, and case management.
- Build robust relationships with Elders, families, community members, and service providers.
