Right at Home Darling Downs is expanding our clinical services and is seeking two compassionate and experienced Clinical Care Partners, with one position based in Dalby and one position based in Warwick.

We welcome applications from Registered Nurses and suitably qualified Allied Health Professionals who have experience in aged care, community care, disability, health services or care coordination.

Both positions will commence on a casual basis, with the potential to transition to a part time or full-time position as our services and client needs in these regions continue to grow.

Your Role As a Clinical Care Partner, you will use your professional knowledge and experience to support the delivery of safe, individualised and person-centred care.

Depending on your professional background and scope of practice, your responsibilities may include:

- Conducting comprehensive client assessments and onboarding
- Developing, implementing and reviewing individualised care plans
- Conducting in-home client visits and assessments
- Monitoring changes in clients' health, wellbeing and care needs
- Identifying concerns and ensuring appropriate clinical escalation or referral
- Providing professional advice and guidance to care coordination and rostering teams
- Liaising with clients, families, GPs, allied health professionals, hospitals and other healthcare providers
- Coordinating referrals and supporting continuity of care
- Providing education and guidance to care staff
- Supporting staff competency and safe care delivery
- Maintaining accurate and timely client documentation
- Contributing to clinical governance, quality improvement and risk management
- Supporting clients and families to understand their care options and available services

For Registered Nurses, duties may also include nursing interventions such as wound care, medication administration, infection control and monitoring of vital signs, in accordance with scope of practice.

You are comfortable working independently, have strong assessment and communication skills,



and enjoy collaborating with clients, families and other health professionals.

Essential Requirements

Applicants must be either:

Registered Nurse

- Current AHPRA registration as a Registered Nurse

OR

Allied Health Professional

- Relevant recognised tertiary qualification in an allied health discipline
- Current professional registration with AHPRA, where applicable, or relevant professional association membership/registration for your discipline

All applicants should also have:

- Experience within aged care, community care, disability, home care or a related health environment
- Strong assessment and care-planning skills
- Excellent written and verbal communication skills
- Strong organisation and time-management skills
- Sound clinical or professional judgement
- Ability to work autonomously and collaboratively
- Confidence using electronic documentation and computer systems
- Current National Police Check
- Current Driver’s Licence
- Reliable and comprehensively insured vehicle
- Ability to travel within the allocated local service area

Experience in community health, aged care, disability services, care coordination, case management or in-home care will be highly regarded. About Right at Home Darling Downs
