About the role:
The successful applicant will demonstrate advanced clinical assessment and formulation skills, sound risk assessment and management capabilities, and a commitment to delivering recovery-oriented, person-centred care. They will possess the ability to work collaboratively with consumers, families, carers, community partners, and external stakeholders to support positive mental health and wellbeing outcomes.
As a Community Clinician,
you will play a pivotal role in the coordination and delivery of comprehensive community-based mental health care. This includes providing clinical case management, conducting holistic assessments, developing team-oriented care plans, facilitating therapeutic interventions, and supporting individuals to access appropriate services and resources within their local community.
The role requires effective communication and partnership with referrers, multidisciplinary treating teams, primary care providers, and community agencies to ensure seamless care transitions and continuity of support. The clinician will foster family-inclusive and systems-informed practice, supporting consumers, families, and carers throughout their recovery journey while promoting safety, wellbeing, and meaningful community participation.
