About the role

You'll travel to clients' homes across Sydney Region &

- Central Coast , delivering hands-on clinical care and assessments. Your visits will focus on dementia and diabetes care, wound management, and general clinical review, helping clients stay safe and independent at home.

Duties

- Deliver skilled in-home clinical care

- Assess and redress wounds

- Manage diabetes care and insulin

- Support clients living with dementia

- Complete clinical assessments each visit

Skills & experience





- Current, unrestricted AHPRA registration

- Home care or community nursing background

- Current police check &
- NDIS worker screening (or willing to obtain)

- Current First Aid/CPR certificate

- Driver's licence and reliable, insured vehicle
