ForHealth Group is seeking a Registered Nurse or Enrolled Nurse to join Kedron Medical Centre in a part time capacity.
The role focuses on Chronic Disease Management, working with GPs and patients to improve outcomes through preventative care and ongoing support.
You will participate in gpmp/tca activities, health assessments and patient education, while using tools like POLAR, PenCAT and HotDocs to identify eligible patients.
A team-oriented, patient-centred team setting is provided.
