BissyCare is seeking a Disabilities Service Officer to join its team to deliver the following responsibilities:
- Developing and implementing individualised care plans tailored to the specific needs of residents.
- Assessing community requirements for health, welfare, housing, employment, training, and other services.
- Monitoring and reporting on residents’ physical and emotional health, ensuring any concerns were promptly addressed with relevant healthcare professionals.
- Assessing community requirements for health, welfare, housing, employment, training, and other services.
- Engaging with families, welfare agencies, government organisations, and private businesses to identify and address pressing community issues.
- Building robust partnerships to ensure a coordinated approach to community support.
- Raising awareness about the resources and services available to individuals and families within the community.
- Ensuring that stakeholders are informed about how to access these resources effectively.
ABOUT YOU
- Minimum Diploma in Community Services, Individual Support, Mental Health or related subject.
- Experience with mental health & behavior is highly desirable.
- Ability to use technology.
- Passionate about helping people.
- Excellent communication skills (written & verbal).
- Excellent documentation skills.
- Must have desire and commitment to ongoing learning, including the need to gain more knowledge and skills to improve their practice.
- NDIS Worker Screening Check.
- Valid Driver’s License.
Pay: $72,000.00 – $80,000.00 per year
