Care Centrix Disability Services is seeking compassionate and dedicated Disability Support Workers to join our team. You will support individuals with disabilities to live independently, achieve their goals, and participate in their communities.

Your role will involve providing person-centred support, assisting with daily living activities, and empowering participants to build skills and confidence.

Key Responsibilities

- Support participants with personal care, meal preparation, and household tasks
- Assist with community access, appointments, and recreational activities
- Promote independence and help participants achieve their NDIS goals
- Maintain accurate documentation and progress notes
- Work collaboratively with participants, families, and allied health professionals
- Follow individual support plans and uphold participants' rights, privacy, and dignity

About You

- Compassionate, patient, and reliable
- Strong communication and interpersonal skills




- Ability to work both independently and as part of a team
- Committed to person-centred and strengths-based support
- Minimum 1 year experience in disability support or related field

Requirements

- Current NDIS Worker Screening Check
- Working With Children Check (WWCC)
- First Aid & CPR Certificate
- COVID-19 Vaccination (as required)
- Driver's licence and reliable vehicle (desirable)
- Certificate III or IV in Disability, Individual Support, Community Services, or equivalent (preferred but not essential)
- Valid Australian work rights required for this position
