Disability Support Worker – Community Access | Thornlie

Core Disability Support Services is seeking a compassionate, reliable and person-centred Disability Support Worker to provide one-to-one community access support to an adult NDIS participant in the Thornlie area.

About the role

This casual position focuses on supporting the participant to increase independence and participate in meaningful community activities. Support will include accessing the local community, attending church and participating in activities aligned with the participant's interests and goals.

The successful applicant will be calm, patient, respectful and reliable, with the ability to develop a positive professional relationship while promoting choice, dignity and independence.

Consistency and reliability are particularly important, as we are seeking a support worker who can build an ongoing professional relationship with the participant.

Key responsibilities

- Provide one-to-one community access and social participation support
- Support attendance at church and other preferred community activities
- Provide safe transport to and from community activities as required
- Encourage independence, choice and decision-making
- Support the participant to develop and maintain community connections
- Follow individual support plans and organisational policies and procedures
- Maintain appropriate professional boundaries
- Complete accurate progress notes and required documentation
- Communicate appropriately with the participant, family and relevant stakeholders

Essential requirements

- Current NDIS Worker Screening Check, or ability to obtain one
- Current First Aid and CPR




- Current WA driver's licence
- Reliable vehicle with comprehensive motor vehicle insurance and willingness to use the vehicle for participant transport
- Good verbal and written communication skills
- Ability to work independently and use initiative
- Reliable, punctual and committed to consistent support
- Understanding of person-centred practice, confidentiality and qualified boundaries

Desirable

- Certificate III or IV in Individual Support, Disability or a related qualification
- Previous experience supporting people with autism and/or psychosocial disability
- Experience providing community access support
- Experience supporting participants to build independence and community participation
