Position Purpose The Disability Support Worker and Mentor provides direct support to NDIS participants (also referred to as ‘clients’), including adults, youth and children, in a variety of community settings and in-home environments.

The role involves providing empowering and personalised support to meet the individual needs of people with disabilities, in alignment with their NDIS goals, support care plans and organisational policies.

This includes, but is not limited to, providing in-home support, personal care, facilitating social and community participation, skill development, personal development and mentoring.

The Disability Support Worker and Mentor respects NDIS participants’ rights to have control over their life and make their own choices and decisions. Through building strong, positive relationships, the Disability Support Worker and Mentor promotes and celebrates NDIS participants’ progress towards a fulfilling and autonomous life.

Position Duties & Responsibilities

Client/Participant Services

Read and understand the needs of each participant prior to commencing shifts to ensure optimal care and support can be provided.

Provide direct care assistance in accordance with participant’s individualised support care plans and as directed by management, including:

In-Home Support: Assist participants with daily living activities, meal preparation, and household tasks.

Social and Community Participation: Facilitate and support participants to access and engage in age-appropriate social activities, community events, and recreational pursuits.

Personal Care: Provide assistance with personal hygiene, dressing, mobility, and medication administration.

Skills-Building: Support participants in developing life skills, including budgeting, cooking, and using public transport, as well as soft skills including confidence building, creativity and problem-solving.

Personal Development: Encourage and assist participants in pursuing educational opportunities, hobbies, and interests.

Mentoring: Build positive relationships, offer guidance, and provide emotional support to enhance participants' well-being and independence.

Maintain positive professional relationships with participants, their families/carers,



other team members and stakeholders.

Work collaboratively with participants, their families/carers, other team members and stakeholders to support the participant’s individual goals and demonstrate and maintain high levels of professionalism, integrity and respect.

KPIs

· Support care plans, documents, individual goals, risk assessments, and any other relevant information is thoroughly reviewed and understood upon acceptance of support/mentoring shifts.

· Support care plans are implemented and adhered to, ensuring participants receive high quality, flexible and person-centred support.

· Shift instructions and previous shift notes are reviewed prior to shift commencement.

· Outcomes are achieved and reflect informed choice by the participant.

· Suggestions to improve or expand on service delivery beyond what is outlined in the support care plan are discussed with management.

· Contribute to case reviews and progress reports where applicable. Attend meetings with families/carers, stakeholders, and other service providers as directed by Creativ Minds management.

· Staff are punctual and shifts are commenced on time. Correct procedures are followed in the event of unforeseen lateness.

· Qualified boundaries are maintained at all times in accordance with organisational policies.

· Communication with families/carers, stakeholders and other service providers is directed to and shared with Creativ Minds management to ensure accurate record keeping and service delivery.

· Staff are reliable, attentive and present while promoting a participant’s independence and autonomy.
