St Vincent's Health Australia is seeking a Grade 2 Social Worker for a fixed term, part time role (3 days per week) based at St Vincent's Hospital Melbourne, Fitzroy.
The role focuses on supporting patients in the cancer and chronic illness team within an inpatient hospital setting.You will conduct psychosocial assessments, coordinate family involvement, and contribute to discharge planning, quality improvement, and qualified education.
