The Role

As the APS 4 Transition Support Officer, you will play a critical role supporting ADF members and their families as they prepare and navigate one of the most significant milestones of their lives, transitioning from military to civilian life.

You will be responsible for delivering personalised support, provide expert guidance and work collaboratively with members, families and a range of stakeholders to ensure successful transition outcomes.

Key responsibilities include:

- Conducting comprehensive face to face support sessions focused on developing and maintaining an individualised Transition Plan, to assist members and their families being prepared for the transition.
- Providing transition support to more complex cases as required.
- Assessing and using judgement to determine member need by providing advice on benefits available to members under the Defence Force Transition Program (DFTP), to assist and prepare members for transition and subsequent civilian life.
- Making referrals to the Commonwealth provider of Transition Support Services as required.
- Supporting Commanders with transitioning members from service.
- Contributing to the delivery of Member and Family Transition face to face Seminars and contribute to the virtual offerings, including verbal delivery of presentations as required.
- Maintaining accurate, timely and compliant records, including detailed case notes and documentation within relevant systems.
- Providing information, guidance and referrals to a broad range of support services to address individual need.
- Undertaking administration and operational tasks as required to meet operational requirements.

About our Team

The Joint Transition Authority (JTA) leads Defence's transition system, ensuring ADF personnel and their families are prepared and supported through all stages of transition, during service and as they move to civilian life. Transition Service Delivery staff are located predominantly in JTA Transition Centres, across major military establishments nationally.





In line with Defence's strategic priorities, JTA is implementing a national operating model that delivers a streamlined and consistent transition experience for all ADF members, regardless of Service. The team is committed to helping members and their families prepare early for transition, build confidence for life after service and access the support, information and opportunities they need to achieve positive outcomes.

We are seeking are seeking a motivated and adaptable professional who is passionate about supporting ADF members and their families through one of the most critical and significant periods of their lives. You will bring a strong customer service focus, with a compassionate and patient approach, and commitment to delivering high-quality support services within a complex and dynamic environment.

As the successful candidate, you will have well developed interpersonal and communication skills, allowing you to build trusting and meaningful working relationships and confidently engage with a range of stakeholders. You will have a strong background in case management/ life coaching or similar, including the ability of writing and maintaining accurate and detailed case notes. You will respond to sensitive matters with empathy, professionalism and sound judgement.

You will also show commitment to ongoing learning and willingness to participate in additional professional development to support in the successful delivery of transition services.

Experience of or knowledge regarding Joint Transition Authority programs and services will be highly regarded. Ex-Military personnel with a minimum of 4 years service (with the last day of service being no more than 5 years prior) who have training and experience in building teams, organisational skills and elements of leadership are encouraged to apply.
