Job Description
Anglicare Sydney is seeking a Care Partner - Villages to support older Australians in accessing care, services and government funding within Warriewood Brook Village.
This permanent, full time, onsite role runs Monday to Friday 8:30am–4:30pm with minimal travel within the village.
You will guide residents and families through their care journey, develop tailored care plans, coordinate funding, and collaborate with village teams and healthcare specialists to ensure high-quality outcomes.
