Apunipima Cape York Health Council is a community controlled Aboriginal and/or Torres Strait Islander health service. Servicing the communities of Cape York, with the main office is located in Cairns, Apunipima delivers public health care, and social and emotional well-being care, to community members. Being a part of the team at Apunipima means helping us deliver essential services as well as building capacity in communities to manage their own health care needs.

About the Opportunity

There is a genuine occuptaional requirement under Section 25 of the Anti-Discrimination Act 1991 (Qld), for this position to be filled by an Aboriginal or Torres Strait Islander person. The position is therefore only open to Aboriginal or Torres Strait Islander applicants.

Reporting to the ITC Program Lead, the ITC Care Coordinator will travel to various Cape York communities and facilitate access to the program for eligible clients with chronic health conditions.

Pre-employment screening for vaccination against Vaccine Preventable Diseases will be conducted for the successful applicant.

Mandatory Requirements

- Current Blue Card positive notice or exemption, or willing and able to obtain prior to commencement
- Current Queensland Drivers Licence
- National Police Check within the last 3 months or willing and able to obtain prior to commencement
- Certificate IV or Diploma in Aboriginal and/or Torres Strait Islander Primary Health Care

About You To be successful in this role, you will demonstrate:

- Experience in chronic conditions management and care coordination
- Experience advocating for access to services on behalf of clients




- Experience working collaboratively with other health care providers to deliver or facilitate access to quality health care for clients
- A working knowledge of clinical best practice in relation to chronic conditions management preferably in remote settings
- Demonstrated understanding of integrated and collaborative client care within a multidisciplinary primary health care team
