Job Description

Disability Support Workers are valued members of Connected Inc NDIS playing a vital role in the delivery of high-quality NDIS services and maximizing the independence of the people we support. The role is responsible for providing direct support such as personal care assistance, community support, social support, and inclusion. This position will also involve supporting independent living skills, domestic activities, improved relationships, and assistance with transport and being a positive role model for participants.

We work with a wide range of people with disabilities with a particular speciality supporting those experiencing Psychsocial Disabilities

Key responsibilities n

- Implement the participant's personal Support Plan in line with the participant's NDIS Goals and personal challenges
n
- Provide community support
n
- Provide social support and inclusion
n
- Support independent living skills
n
- Assist with domestic activities
n
- Support improved relationships
n
- Assist with transport
n




- Work under the direction of the Disability Services Manager to provide efficient and effective day-to-day operations of NDIS services
