We are looking for a passionate and dedicated NDIS Support Coordinator to join our team and support NDIS participants to achieve their goals, build independence, and make the most of their NDIS plans.

About the Role

As a Support Coordinator, you will work closely with NDIS participants, their families, support networks, and service providers to coordinate appropriate supports and help participants achieve their goals.

Key Responsibilities

- Support participants to understand and implement their NDIS plans.
- Coordinate services and supports based on individual participant needs and goals.
- Connect participants with appropriate community and mainstream services.
- Develop and maintain positive relationships with participants, families, providers, and other stakeholders.
- Monitor participant progress and support outcomes.
- Assist participants with service agreements, referrals, and coordination of supports.
- Maintain accurate and confidential participant records and documentation.
- Advocate for participants and promote their choice, control, and independence.
- Ensure all work is completed in accordance with NDIS requirements and relevant legislation.





About You We are looking for someone who:
- Has experience in NDIS Support Coordination or a related community services role.
- Has a robust understanding of the NDIS and participant plans.
- Has excellent communication and organisational skills.
- Is compassionate, reliable, and participant-focused.
- Can work independently and as part of a team.
- Has strong documentation and case-management skills.
- Holds or is willing to obtain a NDIS Worker Screening Check and Working with Children Check, where applicable.
- Has a current Australian driver's licence and reliable transport.

Pay: $40.00 – $60.00 per hour
