We're a local NDIS provider supporting people across Geelong, the Bellarine and Surf Coast. We're looking for an experienced Support Coordinator who enjoys getting to know people, solving problems and helping participants make the most of their NDIS plans.

What you'll do

You'll Work Directly With Participants And Their Families To Understand Their Goals, Find Suitable Services And Keep Supports Running Well. This Includes

- Helping participants understand their plans and budgets
- Connecting them with providers, community services and mainstream supports
- Following up when services aren't working or needs change
- Keeping clear case notes and preparing progress reports
- Supporting participants to make their own choices about providers

About The Candidate You'll have experience in support coordination, case management or a similar disability role,



along with a good understanding of the NDIS. You'll be organised, approachable and confident working through challenges with participants and providers. Knowledge of services around Geelong would be valuable. You'll also need an NDIS Worker Screening Check, Working with Children Check, current driver's licence and access to a reliable vehicle.
