Carbal is looking for an experienced and motivated NDIS Support Coordinator to join our team in Toowoomba.

This is a rewarding opportunity to work directly with NDIS participants, helping them understand their plans, connect with the right services and supports, exercise choice and control, and build greater confidence in navigating the NDIS.

About the Role As an NDIS Support Coordinator, you'll work alongside participants to help them understand and implement their NDIS plans and make informed decisions about the supports and services that are right for them.

You'll help participants identify appropriate providers, connect with community and mainstream services, coordinate supports and work through issues that may be preventing them from achieving their goals.

This is a role for someone who enjoys working with people, solving problems and getting things organised. You'll need to be comfortable managing your own caseload and working independently, while knowing you have the support of the broader Carbal team around you.

You'll also have the advantage of working within a multidisciplinary organisation, with access to GPs, Aboriginal Health Workers, Allied Health professionals, community programs and other support services, while also developing strong relationships with external NDIS providers and community organisations.

What You'll Be Doing Participant Support & Plan Implementation

- Help participants understand their NDIS plans, funding and available supports
- Work with participants to identify priorities and implement their plans
- Support participant choice,



control and informed decision-making
- Help participants identify and engage providers that meet their individual needs and goals
- Build participant confidence and capacity to navigate the NDIS and their supports
- Monitor progress and assist participants to respond when their needs or circumstances change

Service Coordination

- Connect participants with NDIS providers and supports
- Assist participants to access appropriate community, mainstream and government services
- Coordinate services across multiple providers where required
- Work with participants, families, informal supports and providers to help ensure supports work effectively together
- Assist in resolving service issues and barriers to accessing support

Advocacy & Relationships

- Advocate alongside participants where required
- Build respectful and meaningful relationships with participants and families
- Develop strong working relationships with NDIS providers and community organisations
- Work collaboratively with Carbal's multidisciplinary health and community teams
- Promote participant independence, dignity, choice and control

Caseload & Documentation

- Manage and prioritise your participant caseload
- Maintain accurate records of supports and participant activity
- Prepare reports and documentation as required
- Maintain appropriate professional boundaries and confidentiality
- Meet NDIS, organisational and reporting requirements

We're looking for someone who genuinely enjoys helping people navigate systems, solve problems and access the supports they need.

Essential

- Experience in NDIS Support Coordination, disability, community services, mental health, case management or a closely related field
- A sound understanding of the NDIS, or demonstrated ability to quickly develop this knowledge
- Understanding of person-centred practice, participant choice and control
- Strong communication and relationship-building skills
- Ability to work independently and manage your own workload
- Strong organisational skills and ability to prioritise competing caseload demands
- Good written communication, report writing and documentation skills




- Ability to maintain accurate participant and service records
- Understanding of how social, emotional and physical wellbeing can affect a person's life and relationships
- Current C Class Driver Licence
- Current NDIS Worker Screening Clearance and Blue Card, or eligibility to obtain the required clearances
- Commitment to culturally safe, respectful and person-centred practice

Highly Regarded

- Previous NDIS Support Coordination experience
- Qualification in Disability, Community Services, Human Services, Social Work, Mental Health or a related discipline
- Experience in the disability, community health, mental health or not-for-profit sectors
- Experience developing or implementing individual support plans
- Experience working respectfully with Aboriginal and Torres Strait Islander people and communities
- Understanding of Aboriginal and Torres Strait Islander culture and culturally safe service delivery
