We are seeking an experienced and passionate Practice Advisor - CYF Incident Review to join our Children, Youth and Families team.

This is an opportunity to make a real impact behind the scenes, helping ensure children and young people receive safe, high-quality support. You'll bring your practice expertise, sound judgement and curiosity to help us learn from incidents, strengthen practice and continuously improve the way we work.

About you and your new opportunity:

Reporting to the CYF Operations Manager, you will play a critical role in supporting and guiding leaders across our services. Collaborating with operational teams Statewide, Service Leadership Teams, the CYF Practice Team, and the Quality & Compliance Team, you will analyse incidents, identify risks and patterns, and promote a culture of learning and continuous improvement. Your work will contribute to safer, more effective services and better outcomes for children and young people.

In this role, you'll:

- Oversee incident management processes across our Children, Youth & Families Services.
- Manage and review Standards of Care Reviews (SOCRs), Harm Reports, Reliable Incident Reports, allegations of misconduct, and other high-risk incidents.
- Provide expert advice to Service Managers and Team Leaders on incident management, risk assessment, and escalation requirements.
- Work collaboratively with Child Safety Service Centres and other key stakeholders.
- Identify recurring themes, emerging risks and opportunities for improvement.
- Monitor and follow up on incident management actions and support organisational learning.




- Use data and insights to support organisational learning and continuous improvement.
- Contribute to Nominee reporting and broader risk management activities.

What you will bring to our team: You are a skilled and reflective practitioner who combines sound judgement and strong risk assessment capabilities with a commitment to child safety, quality practice, and continuous improvement. You thrive in collaborative environments, confidently navigate challenging conversations, and are passionate about driving positive outcomes for children and young people.

You will have:

- A degree in Social Work, Psychology, Social Sciences, or a related discipline.
- Demonstrated experience working within child protection, out-of-home care, or statutory care environments.
- Strong knowledge of child protection legislation, trauma-informed practice, and child development.
- Experience supporting children, young people and families with complex and multiple support needs.
- A sound understanding of risk assessment, incident management, investigations, and quality improvement processes.
- Experience working alongside Aboriginal and Torres Strait Islander peoples, communities and colleagues in a culturally safe and respectful way.
- Well-developed communication, report writing, consultation and relationship-building skills.
- The ability to analyse information, identify trends and make practical recommendations.
- Intermediate Microsoft Office skills.

It would be great if you also have:





- Certificate IV in Training and Assessment (or equivalent).
- Experience developing policies, delivering training or participating in quality improvement initiatives.
- Experience using learning management systems or eLearning platforms.

Essential for the role: Child Safety is everyone's responsibility. Our robust screening and recruitment processes help ensure we engage the right people to support children and young people. This role will require the following to be obtained before commencing:

- Current Driver's Licence.
- National Police Certificate (or ability to obtain).
- Blue Card / Working with Children Check (or ability to obtain)
- Child Safety and Personal History Screening Check (LCS) (or ability to obtain).
