Care Partner - Full Time Mareeba (Admin Role)

As a Care Partner, you will coordinate services that support older people and people with disability to live safely and independently in their homes and communities. You will work closely with clients, families, support workers and other stakeholders to develop tailored care plans, manage service delivery, coordinate staff and ensure high-quality outcomes.

Key responsibilities

n

Build and maintain relationships with referral partners, community stakeholders and service providers.

n

Contribute to client growth and service engagement opportunities within the community.

n

Conduct client assessments and develop person-centred care plans.

n

Coordinate and support a team of Care Workers.

n

Manage client services within funding and budget requirements.

n

Complete regular client reviews and home visits.

n

Monitor risks and implement appropriate support strategies.

n

Maintain accurate documentation and client records.

n

Assist with recruitment, onboarding and staff development.

n

Ensure compliance with NDIS, Aged Care and organisational standards.

n

Respond to client, family and staff enquiries professionally and promptly.

n

About you

n

A proactive, persistent, and self-motivated approach, with the ability to take initiative and follow through on tasks.

n

Certificate III in Individual Support, Community Services, Business, or an equivalent qualification. A completed Certificate III in Individual Support (or a higher relevant qualification)



will be highly regarded.

n

Experience supporting older people and/or people with disability is desirable.

n

Excellent communication and interpersonal skills, with the ability to build positive relationships with clients, families, and stakeholders.

n

Previous sales, business development, customer service, client engagement, or relationship management experience, particularly within a community care, aged care, disability support, or health services workplace, will be highly regarded.

n

Sound knowledge of NDIS and/or Aged Care standards and service delivery models.

n

Strong technology skills, including confidence using mobile apps, client management systems, and digital communication tools.

n

Current Queensland Driver's Licence and access to a reliable vehicle.

n

Current NDIS Worker Screening Check, Police Check and Blue Card (or the ability to obtain).
