We have an exciting opportunity for a qualified Registered Nurse to join our In-Home Care team as a Care Partner - Clinical to manage and oversee high-quality care delivery for our consumers in their own home. You will aim to achieve the best care outcomes for each consumer from their funded aged care services.

The Role:

Reporting directly to the General Manager In-Home Care you will be responsible for managing the entire lifecycle of Home Care Packages (HCP) and Support at Home (SaH) including intake of current clients and managing their ongoing care while providing excellent customer service for consumers and adhering to the Aged Care Standards.

Key Responsibilities:

- Care planning and coordination
- Review, assess and evaluate the provision of care
- Develop and maintain care plans and budgets aligned to individual aged care funding
- Liaise with relevant service providers

Qualification:

- Registered Nurse Div 1 with current AHPRA registration (no conditions)

Licence:

- Current Victorian Drivers Licence

Location:

Ideally you will live in Moonee Valley, Maribyrnong or Brimbank as our clients are in the West and North of Melbourne.

Experience and skills:

- Experience in working in a community aged care setting.
- Clinical knowledge that encompasses clinical assessment, prevention, planning, treatment, management, and review to optimise quality of life, reablement and maintenance of older persons living in the community.
- Extensive knowledge of Commonwealth Home Care Packages.
- Strong planning, time management and organisational skills with the ability to prioritise accordingly.




- Sensitivity in addressing consumers' emotional and physical needs.
- High level interpersonal and written skills.
- Efficient time management.
- Proficiency in Microsoft Office (Word and Excel); knowledge of Alayacare is highly desirable
