About the Role

Community Home Care Services is seeking a compassionate, experienced and dedicated Registered Nurse (RN) to join our team in Chester Hill.

This is a full-time fixed-term position providing maternity leave cover for an initial period of 1–2 years, with the possibility of extension/ fix term position subject to organisational requirements and performance.

This is a full-time prospect for a nurse who is passionate about providing high-quality, person-centered care and supporting clients to remain safe, independent and comfortable in their own homes.

As a Registered Nurse, you will be responsible for providing professional nursing care to clients in their homes and community settings. Your key responsibilities will include:

- Conducting comprehensive nursing assessments and developing individualised care plans.
- Providing high-quality clinical care in accordance with each client's needs and care plan.
- Monitoring clients' health, wellbeing and changes in clinical condition.
- Administering medications and providing medication management as required.
- Wound care, continence care and other nursing interventions within your scope of practice.
- Liaising with GPs, allied health professionals, hospitals, families and other healthcare providers.
- Maintaining accurate and timely clinical documentation and progress notes.
- Supporting clients to achieve their health, wellbeing and independence goals.
- Providing education and guidance to clients, families and carers.
- Identifying and escalating changes in a client's condition or potential health and safety risks.




- Ensuring all care is delivered in accordance with relevant legislation, professional standards, organisational policies and procedures.
- Contributing to continuous improvement and maintaining a high standard of client-focused care.

What We Expect From You We are looking for someone who is:

- A Registered Nurse with current AHPRA registration.
- Experienced in community, home care, aged care or a similar healthcare environment.

- Organised, reliable and able to manage competing priorities.
- Professional and committed to maintaining client confidentiality and dignity.

- Committed to maintaining professional development and current clinical knowledge.
- Able to work in accordance with workplace health and safety requirements.
