About the Role n

In this role, you will:

n

- Prepare and coordinate Chronic Condition Management Plans and Reviews
n
- Support patients in managing chronic health conditions and achieving positive health outcomes
n
- Conduct health assessments and preventative health activities
n
- Work closely with GPs and the wider multidisciplinary healthcare team
n
- Provide patient education and ongoing support
n
- Maintain accurate clinical records and appointment administration
n

About You n

To be successful in this role, you will have:

n

n
- Current AHPRA registration as a Registered Nurse or Enrolled Nurse
n
- Previous experience in Chronic Condition Management (highly regarded)
n
- Strong communication and interpersonal skills
n
- Excellent organisational and time management abilities
n
- A proactive, patient-focused approach to care
n
- The ability to work both independently and collaboratively within a team
n
