About the Role Carbal is looking for a Social Worker to join our Growing Deadly Families team, supporting Aboriginal and Torres Strait Islander women, babies, children and families throughout pregnancy, birth and the early parenting period.

This is a varied, relationship-focused role combining psychosocial support, assessment, case management, advocacy and practical assistance. You'll work directly with women and families to understand their individual circumstances, build on their strengths and help address the social and practical issues that can affect health and wellbeing.

You may support families experiencing challenges such as social isolation, family stress, housing instability, financial hardship or other complex circumstances. You'll help connect clients with appropriate health, social, housing, financial, family and community services and advocate for them when barriers to accessing support arise.

Importantly, you won't be doing this work in isolation. You'll work closely with GPs, Midwives, Aboriginal Health Workers, Practice Nurses, Allied Health professionals, our GDF Cultural Liaison Officer and community organisations, contributing to coordinated, culturally safe care around each woman and family.

What You'll Be Doing Psychosocial & Family Support

- Undertake psychosocial assessments and identify individual and family strengths, needs and support requirements
- Develop support plans in partnership with clients and the multidisciplinary team
- Provide strengths-based, trauma-informed and family-centred support
- Support women and families experiencing social, emotional, family or environmental challenges
- Promote resilience, independence and positive family wellbeing

Case Management & Advocacy

- Coordinate support for clients with complex needs




- Advocate for women and families to support equitable access to services
- Connect clients with health, housing, financial, family support and community services
- Coordinate referrals and follow-up
- Help clients navigate health, government and community support systems
- Maintain ongoing engagement throughout pregnancy and early parenting

Maternal & Family Wellbeing

- Support women and families throughout pregnancy, birth and the postnatal period
- Promote positive parenting, healthy lifestyles and family wellbeing
- Provide health education and practical support
- Encourage participation in preventative health and early intervention
- Help women and families remain connected with health services

Multidisciplinary & Community Work

- Work collaboratively with GPs, Midwives, Aboriginal Health Workers, Nurses, Allied Health professionals and the GDF Cultural Liaison Officer
- Participate in multidisciplinary case discussions and coordinated care planning
- Build productive relationships with community organisations and external service providers
- Participate in community engagement, outreach, education and health promotion activities
- Maintain accurate case notes, client records and program documentation
- Contribute to continuous improvement of the Growing Deadly Families Program

We're particularly interested in hearing from qualified Social Workers who enjoy relationship-based practice and want to use their professional skills in a multidisciplinary community health environment.

Qualifications & Experience

Ideally, you'll have:

- A Bachelor of Social Work or recognised equivalent qualification
- Eligibility for membership of the Australian Association of Social Workers (AASW)
- Experience providing psychosocial support, case management, advocacy, counselling or family support
- Understanding of trauma-informed, strengths-based and person-centred practice
- Strong interpersonal, communication, advocacy and relationship-building skills
- Ability to work independently while also contributing effectively within a multidisciplinary team
- Strong organisational and case-management skills
- Ability to maintain professional boundaries, confidentiality and accurate documentation
- Commitment to culturally safe and respectful practice with Aboriginal and Torres Strait Islander women, families and communities

Other Professional Backgrounds Welcome While Social Work is our preferred professional background, we recognise that relevant skills and experience can be developed through other disciplines.

We also welcome applications from experienced professionals with a relevant tertiary qualification in Human Services, Community Services, Psychology, Counselling, Behavioural Science or another closely related discipline, particularly where you can demonstrate strong experience in psychosocial support, case management, advocacy and family-centred practice.

You'll Also Need

- Current Open C Class Driver Licence valid in Queensland
- Current Blue Card, or eligibility to obtain and maintain one
- National Police Check
- Current CPR certification, or willingness to obtain

Highly Regarded

- Experience in maternal health, child health, family wellbeing or perinatal mental health
- Experience within an Aboriginal Community Controlled Health Organisation or community health setting
- Experience working respectfully with Aboriginal and Torres Strait Islander people, families and communities
- Knowledge of primary health care, early intervention and child development
- Knowledge of housing, family support, domestic and family violence and community support systems
- Mental Health First Aid or other relevant professional qualifications

These requirements are consistent with the existing GDF role, including the preference for AASW eligibility where applicable and experience in maternal, child or family wellbeing programs.
