Employment with Mid North Coast LHD requires candidates to hold a valid authority to work in Australia (Aust Work Rights Visa).

What you'll be doing To provide an effective and efficient Social Work service that addresses the psychosocial aspects of care for patients as identified, providing intervention including support, education and resources to assist in dealing with health events.

To actively participate and contribute to the continued development of the Social Work professional role in the context of Health.

This position will support IMHAOD services.

About You

You are a dedicated and highly skilled Social Worker with qualifications eligible for membership of the Australian Association of Social Workers and extensive experience supporting individuals, families, and communities within complex healthcare environments. You bring a robust understanding of the social determinants of health and are committed to delivering compassionate, person-centred care that enhances patient wellbeing and supports positive health outcomes.

Your expertise in psychosocial assessment, care planning, and intervention enables you to effectively manage complex presentations, identify individual and family needs, and coordinate appropriate supports across the continuum of care. You are confident working with patients experiencing significant health and social challenges, applying evidence-based approaches to advocate for their needs and facilitate meaningful outcomes.





A collaborative and proactive practitioner, you work effectively alongside multidisciplinary teams, community organisations, and external agencies to ensure integrated and holistic care. You have demonstrated experience contributing to the development, implementation, evaluation, and continuous improvement of social work services, supporting both service excellence and patient-centred outcomes.

Known for your exceptional communication, interpersonal, and organisational skills, you are able to manage a demanding workload while maintaining high standards of professionalism and accuracy. Your strong written, verbal, and computer skills support effective documentation, reporting, and stakeholder engagement, allowing you to make a valuable contribution to healthcare teams and the delivery of quality social work services.
