Care Guidance exists to empower all families to find the right care for their loved ones.

Our Wellbeing Program provides social-work-led support to older Australians across residential aged care, Support at Home and hospital settings.

We currently work with residential aged care providers across Melbourne and are continuing to expand our Wellbeing Program. We are looking for experienced Social Workers to join our existing team of more than 30 Social Workers.

Our ambition is to make social-work-led wellbeing support a core part of aged care across Australia, helping address the emotional, mental health and psychosocial needs that can often be overlooked as people enter and live in aged care.

About the role

We are looking for experienced Social Workers to Attend one or more dedicated residential aged care homes on a weekly or fortnightly basis, providing continuity of support to residents.

You will work closely with residents, families and facility teams to identify needs and provide ongoing social work support.

Depending on the home and resident needs, your work may include:

- Supporting residents adjusting to entering residential aged care




- Social isolation and loneliness
- Grief, loss and bereavement counselling
- Mental health and emotional wellbeing support
- Psychosocial assessments
- Family and carer support
- Advanced care planning
- Supporting residents experiencing complex psychosocial issues
- Advocacy and assistance navigating services
- Working alongside clinical and care teams to identify residents who may benefit from social work support

The role offers the opportunity to work with residents on an ongoing basis, allowing you to build meaningful relationships and provide continuity of support. Depending on your availability and interests, there are also opportunities to provide Support at Home services, visiting older Australians in their homes for one-off or ongoing social work support, as well as work with Care Guidance’s partner hospitals.

Skills and experience

- Bachelor of Social Work
- Approximately 5+ years of relevant social work experience
- Eligibility for membership with the Australian Association of Social Workers (AASW)
- Police check
- Social work experience within aged care, health, mental health and/or palliative care

Pay: From $58.00 per hour Expected hours: 7.5 per week
