Position Overview The Social Worker at Thoughts Australia plays a critical role in delivering person-centered support to individuals with disabilities, their families, and carers. The position focuses on assessing client needs, developing support strategies, and facilitating access to appropriate services in alignment with NDIS frameworks and organizational objectives.

Working within a multidisciplinary environment, the Social Worker contributes to improving participant wellbeing, promoting independence,



and advocating for the rights and inclusion of individuals within the community.

Key Tasks and Responsibilities

- Assess clients’ needs, risks and circumstances using psychological frameworks, and develop appropriate intervention and support plans.
- Provide counselling, advocacy and crisis intervention to individuals, families and carers to improve wellbeing and social functioning.
- Develop, implement, and review case management plans, including safety plans, behavioral strategies, and support pathways.
- Liaise and collaborate with multidisciplinary teams, healthcare providers, government agencies and community services to coordinate support.
- Advocate for clients’ rights, access to services, and equitable outcomes within the NDIS and broader community systems.
- Monitor and evaluate client progress, maintaining accurate case notes, reports, and documentation in line with regulatory standards.
- Provide professional supervision, guidance and training to staff to ensure quality, ethical, and evidence-based practice.
- Ensure compliance with relevant legislation, including the NDIS Act, Practice standards, and safeguarding requirements.
- Contribute to program development, service improvement, and implementation of social work practice frameworks.
- Participate in organizational planning, policy development, and quality assurance processes to enhance service delivery

Qualifications/Skills Required

- Bachelor’s degree or a higher qualification in a relevant field
- Eligibility for membership with the Australian Association of Social workers (AASW)
- At least one year of relevant experience
- Strong knowledge of the NDIS Act and standards
- Demonstrate understanding of social work theories, frameworks
