About the Role

An exciting opportunity exists for an experienced Grade 2 Physiotherapist to join our Rehab @ Home team at St Vincent’s Hospital Melbourne. In this role, you will provide high-quality physiotherapy assessment, treatment and management to patients receiving rehabilitation in their own homes. Care is delivered through a combination of face-to-face visits and virtual care, supporting patients to achieve their rehabilitation goals and enabling earlier, supported discharge from hospital. Working as part of a collaborative interdisciplinary team, you will manage a varied caseload while contributing to the ongoing development and improvement of the Rehab @ Home service

Your responsibilities will include:

- Providing comprehensive physiotherapy assessment, treatment and rehabilitation to patients within the Rehab @ Home program
- Delivering care through both face-to-face and virtual models
- Working collaboratively with patients, families, carers and the broader interdisciplinary team to support individual rehabilitation goals
- Providing supervision and support to physiotherapy students and designated allied health staff
- Participating in quality improvement initiatives and service development activities
- Contributing to a positive, collaborative and patient-centred team environment

Classifications for this position will range from VB1–VB4 ($90,000 - 104,000 per annum,



pro rata for part time).

Your Contribution

You are an experienced and motivated Physiotherapist who enjoys working collaboratively and providing patient-centred care within a community-based rehabilitation setting.

You will bring:

- A recognised qualification in Physiotherapy
- Current registration with AHPRA
- A current Victorian Driver Licence
- A minimum of three years' recent physiotherapy experience
- Demonstrated knowledge and experience in physiotherapy management within a subacute and/or community setting
- Strong communication, clinical reasoning and multidisciplinary teamwork skills

About Rehab @ Home

Rehab @ Home is one of the programs within St Vincent’s Care at Home, providing rehabilitation to a diverse range of patients in their own homes. The service supports earlier discharge from hospital for patients who require ongoing restorative and supportive rehabilitation following illness or injury. Through a combination of face-to-face and virtual care, the program helps patients continue their recovery in a familiar home environment while remaining connected to specialist clinical support. Rehab @ Home operates seven days a week and is delivered by an interdisciplinary team, with qualified support and supervision coordinated through the SVHM Physiotherapy Department.
