Role Description The Support Coordinator – Horsham is a full-time, on-site role based in Horsham, VIC. In this position, the coordinator works directly with clients to understand their needs, develop support plans, and connect them with appropriate services and resources. Day-to-day tasks include managing client cases, scheduling and attending meetings, maintaining accurate records, and following up on service delivery to ensure goals are met.

The role involves liaising with families, external providers, and community organizations, responding to inquiries, and providing clear information and guidance.

The Support

Coordinator is also responsible for using relevant systems and tools to document interactions, monitor progress,



and report on outcomes in a timely and professional manner.

Qualifications

- Strong interpersonal skills and the ability to build respectful, trust-based relationships with clients and stakeholders.

- Effective verbal and written communication skills, including the capacity to explain information clearly and advocate for client needs.

- Experience in case management, coordination of services, or similar roles involving care planning and follow-up.

- Customer service skills with a focus on responsiveness, empathy, and problem-solving in client-facing situations.

- Ability to provide basic technical support related to client records, scheduling systems, and communication tools.

- Relevant qualification in social work, community services, psychology, disability support, or a related field (or equivalent experience).

- Knowledge of local community services and support networks, particularly within Horsham and the broader VIC region.

- Organizational skills, attention to detail, and the ability to manage multiple cases and priorities concurrently.
