Case Management RN Job Details

- Location: Detroit, MI 48202 (Michigan nursing jobs)

- Setting: Inpatient Case Management – Acute Care Hospital

- Assignment Type: Travel RN (Registered Nurse)

- Start Date: November 11, 2025

- End Date: February 7, 2026

- Duration: 13-week travel contract

- Schedule: 8-hour day shifts (8:00am – 4:30pm), rotating weekends

- Hours per Week: 40 guaranteed

- Case Load: 1:21-25 (assignments based on patient risk factors)

Case Management RN Qualifications

- Minimum 5 years of acute inpatient case management RN experience (telephonic, utilization management, or therapy experience not qualifying)

- Recent (within last 2 years) acute inpatient case management experience required

- Proficient computer skills; Epic EMR experience required

- Associate or Bachelor’s degree in Nursing (RN) or Master of Social Work (MSW)

- BLS certification through AHA

- Active Michigan RN license or LMSW with valid Michigan licensure

- Demonstrated acute discharge planning experience

- Excellent verbal communication, documentation, and interpersonal skills

- Strong analytical, problem-solving,



and decision-making abilities

- Knowledge of social determinants of health and community healthcare resources

- Ability to work independently, adapt quickly, and thrive in new healthcare environments

Case Management RN Responsibilities

- Perform comprehensive patient and family assessments to identify medical, psychological, social, and functional needs

- Develop and implement individualized, patient-centered care plans to optimize clinical outcomes

- Engage patients and families in care coordination through advocacy, education, and resource facilitation

- Utilize critical thinking, motivational interviewing, and self-management strategies to address barriers to care

- Coordinate safe and efficient transitions of care between healthcare settings, ensuring continuity

- Advocate for appropriate healthcare services within patient benefit structures

- Collaborate with interdisciplinary teams to manage patients with complex medical and social needs

- Participate in process improvement initiatives to enhance patient safety and outcomes

- Facilitate interventions in cases of abuse, neglect, or complex social situations

- Support department goals and perform additional duties as assigned
