It's one of the most significant transformations taking place in Australian healthcare. /n /n /n About the Opportunity /n We're seeking experienced Clinical Nurses to join our expanding Hospital in the Home (HITH) service across Victoria. /n /n As a HITH Clinical Nurse, you'll deliver advanced clinical care to patients with acute and complex healthcare needs while working as part of a progressive multidisciplinary team committed to innovation, compassion, and clinical excellence. /n /n You'll use your advanced nursing expertise, clinical judgement, and care coordination skills to help patients receive safe, high-quality care in the comfort of their own homes. /n /n Key Responsibilities /n /n
- /n Conduct comprehensive patient assessments and clinical reviews. /n
- /n Develop and implement individualised care plans. /n
- /n Deliver advanced nursing interventions including IV therapies and complex clinical care. /n
- /n Monitor patients with acute and complex healthcare needs and escalate care when required. /n
- /n Coordinate referrals, admissions, and discharge planning. /n
- /n Collaborate closely with Medical Officers, Clinical Leads, and multidisciplinary teams. /n
- /n Support patients, families, and carers through education and care planning. /n
- /n Participate in quality improvement initiatives and service development activities. /n
- /n Mentor and support new and junior team members. /n
- /n Contribute to innovative healthcare delivery that strengthens the connection between hospital and home. /n /n /n /n About You /n You're an experienced Registered Nurse who thrives in an autonomous environment, enjoys building meaningful patient relationships, and is passionate about delivering exceptional clinical care in community settings. /n /n You Will Bring /n /n
- /n Current registration as a Registered Nurse with AHPRA. /n
- /n Minimum three (3) years post-graduate nursing experience. /n
- /n Experience in community nursing and/or acute medical, surgical, emergency, or critical care settings. /n
- /n Demonstrated experience supporting patients with complex clinical needs and multiple co-morbidities. /n
- /n Strong clinical assessment, care planning, and decision-making capabilities. /n
- /n Excellent communication and stakeholder engagement skills. /n
- /n Confidence working independently while collaborating with multidisciplinary teams. /n
- /n Strong digital literacy and willingness to utilise telehealth and virtual care technologies. /n
- /n Current unrestricted Victorian Driver's Licence. /n
- /n Full and unrestricted working rights in Australia. /n /n /n Highly Regarded /n /n
- /n Hospital in the Home (HITH), community nursing, outreach nursing, or virtual care experience. /n
- /n Postgraduate nursing qualifications. /n
