Responsibilities:

- Assist Encara in providing essential clinical services to community clients.
- Ensure timely completion of documentation and reporting as per individual client requirements.
- Receive dedicated administrative support for seamless workflow.
- Engage in case conferencing, multidisciplinary reviews, and case discussions.

We are seeking a candidate with proficiency in the NDIS, HCP, and Aged Care domains. The ideal candidate will excel in building meaningful relationships, managing a dynamic caseload with a practical approach, and demonstrating unwavering commitment to helping clients achieve their optimal outcomes.
