Uplift Community Care Solutions is looking for a Service Manage to oversee our day to day services, support our workers and build strong relationships with participants and their support networks. Working closely with our Director Emily, The Service Manager will oversee our Service Coordinators, Social Worker and Support Workers while bringing a strong NDIS background.

You’ll bring people, rosters and information together so participants receive reliable support that reflects their needs, goals and preferences.

Uplift Community Care Solutions provides services across South Australia to 45 participants with a range of different support requirements. We work with heart, compassion and value positive and flexible attitudes.

What you’ll be doing

- Oversee day-to-day service delivery, ensuring supports reflect participants’ needs, goals and preferences
- Lead and supervise support workers, including performance management and staff development
- Build strong relationships with participants, families, support coordinators and other professionals
- Oversee referrals, participant onboarding, service agreements and changes to supports
- Monitor rosters, staffing levels and worker matching to maintain reliable services
- Manage participant concerns, complaints and incidents, escalating significant matters to the Director
- Maintain oversight of support plans, risk assessments and behaviour support plan implementation




- Monitor compliance with NDIS requirements and Uplift’s policies and procedures
- Work within participant budgets and agreed staffing budgets
- Identify service improvements and report key issues, progress and priorities to the Director

What you’ll bring

- Experience in Community NDIS services, ideally in service coordination, HR, or a similar role
- An understanding of the NDIS, participant rights, safeguarding and person-centred support
- Strong organisation skills and the ability to manage changing priorities
- Clear communication and confidence having sensitive conversations
- A calm, practical approach to solving problems
- Confidence using rostering and record-management systems
- A relevant qualification in disability, community services, social work or a related field is desirable

Relevant worker screening checks and a current driver’s licence will be required.
