About the Role

As a Support Coordinator, you will play a vital role in assisting participants to understand, implement, and maximise their NDIS plans. You will work collaboratively with participants, families, carers, service providers, and community networks to build capacity and achieve meaningful outcomes.

Key Responsibilities

- Assist participants to understand and implement their NDIS plans.
- Coordinate and connect participants with appropriate NDIS, community, mainstream, and informal supports.
- Build participant capacity to exercise choice, control, and independence.
- Support participants to achieve their identified goals and aspirations.
- Develop and maintain strong stakeholder relationships.
- Monitor service delivery and participant outcomes.
- Conduct plan reviews and prepare progress reports.
- Maintain accurate, timely, and compliant case notes and participant records.
- Identify risks and implement appropriate safeguarding measures.
- Ensure compliance with the NDIS Practice Standards, NDIS Code of Conduct, and organisational policies and procedures.

About You





To be successful in this role, you will possess:

Essential Criteria

- Minimum 2 years' experience as a NDIS Support Coordinator.
- Demonstrated knowledge and understanding of the NDIS, participant plan implementation, and support coordination practices.
- Robust understanding of person-centred, strengths-based, and recovery-oriented approaches.
- Exceptional communication, advocacy, negotiation, and relationship-building skills.
- Experience supporting participants with complex needs, psychosocial disability, and diverse cultural backgrounds.
- Excellent organisational and time management skills with the ability to manage competing priorities.
- High-level report writing, documentation, and case management skills.
- Ability to work independently while contributing positively to a collaborative team environment.

Qualifications

- Certificate IV, Diploma, or Degree qualification in Community Services, Disability, Social Work, Psychology, Human Services, or a related field.

Mandatory Requirements

- 100 points ID
- Current NDIS Worker Screening Check.

- Current Working with Children Check.
- Current Australian Driver's Licence and access to a reliable vehicle.
- Current First Aid and CPR Certificate or willingness to obtain.
- Infection Prevention and Control Certificate
- 4 x NDIS mandatory eLearning modules
