Active Allied Health Australia is growing, and we are looking for a qualified and experienced Physiotherapist to join our multidisciplinary allied health team.

We have an opportunity for a community-based Physiotherapist providing services across the Western and Northern suburbs of Melbourne.

About the Role

As a Community Physiotherapist, you will provide physiotherapy services to NDIS participants and Home Care clients, including Support at Home (SAH) across the Western and Northern suburbs of Melbourne.

Conduct comprehensive assessments to develop individualised treatment plans for patients, NDIS participants and HCP clients.

Implementing therapy programs for various conditions, including neurological disorders, musculoskeletal injuries, post-surgical rehabilitation,



and chronic disease and pain management.

Provide hands‑on physiotherapy interventions, such as manual therapy, exercise prescription, and client education.

Providing fall prevention strategies and mobility aid prescriptions (e.g., walkers, orthotics,wheelchairs).

Collaborate with the multidisciplinary team to ensure coordinated and holistic care.

Monitor patient progress and adjust treatment plans for NDIS and HCP clients accordingly.

Maintain detailed client records and ensure compliance with all regulatory and company policies.

Participate in continuous professional development to stay up-to-date with the latest physiotherapy practices.

About You

We are looking for a Physiotherapist who is skilled, reliable and has strong communication and clinical reasoning skills.

You will ideally have:

A relevant Physiotherapy qualification

Current AHPRA registration

Experience in working with Geriatric, people with special needs, preferably community or mobile-based

Ability to work independently and manage a caseload

Strong clinical documentation and communication skills

A current full Australian driver's licence and reliable vehicle

Experience with NDIS, Home Care (Support at Home), rehabilitation, mobility assessment, falls prevention or community physiotherapy will be highly regarded.
