Northside Community Service is seeking a Consumer Intake Officer to join our Entry & Temporary Supports team in Canberra. You will be the first point of contact for clients, guiding referrals through to service delivery across aged and community care programs.

You will assess referrals, identify client strengths and risks, develop support plans, and coordinate service delivery with internal and external providers to keep care plans on track as needs change.
